  • Ensure that all information entered into your online application form is correct and there are no spelling mistakes.

    • ​​​Contact our Tournament Treasurer Lynda Gall by email at treasurer@liftlockatomhockey.ca to receive instructions on what to enter as the password for the e-Transfer
    • Login to your online banking software and create a new e-Transfer
      • Enter your team's Manager name and Head Coach name in the message section.
      • Enter the text "Team Entry Fee" along with your Team Name (i.e. Peterborough Petes) as the security question
      • Enter the password supplied by our Tournament Treasurer Lynda Gall
    • Send e-Transfer to: treasurer@liftlockatomhockey.ca
  2. Cheque or Money Order​
    • Payable to:  Peterborough Liftlock Atom Hockey Tournament
    • ​Mail to:  Lynda Gall, 150 DeClair Road Madoc, On K0K 2K0
    • Non-Sufficient Fund (NSF) Cheques – A bank fee or surcharge of $20.00 in addition to entry fee.
    • Cheque or Money Order must be recieved before your team is accepted into the tournament.
REFUND POLICY
Any team accepted by the Tournament Committee; withdrawing after December 1, 2024 will forfeit their entry fee.

Waiver

Submitting this Entry Form, the team representative, on behalf of his/her team releases the sponsors, officials, arena management and all concerned with this tournament from any liability for any accident or injury which may be incurred by any player, team official or spectator while participating in, coming to or leaving from the tournament . 

Photo and Video Consent
In compliance with the Municipal Freedom of Information and Protection of Privacy Act, the Peterborough Liftlock Atom Hockey Tournament requires permission to allow photo/video recording of identifiable players and coaches.

I authorize the Peterborough Liftlock Atom hockey tournament to use photographs and names of players, coaches, and other team officials on the Peterborough Liftlock Atom hockey Tournament website and other media publications.

Privacy Policy

This personal information is being collected by the Peterborough Liftlock Atom Hockey Tournament will be used to facilitate the tournament and will not be used or disclosed for other purposes, unless permitted by The Freedom of Information and Protection of Privacy Act (FIPPA).
